I'd like to request disclosures for family court proceedings on behalf of my client/local authority
Thank you. To apply, follow these steps:

Step 3: Complete the application form, using the relevant guidance notes to help you.
Step 4: In private law proceedings, you need to make payment at GOV.UK before submitting a request for disclosure. Fees are outlined in the Annex 1 form. Please include the Court Order reference as the payment reference. Note that split payments cannot be accepted. In public law proceedings, there is currently no charge.
Step 5: Email your completed form to the Data Protection team. In private law proceedings, please quote the Court Order reference and payment reference from GOV.UK and attach the ‘Private Law Police Disclosure Order’ (Annex 5), which must be obtained from the court. Failure to do so will result in a delay to your application.
